What changes did you make and why did you make them?

• Carefully replaced HTML with Material UI after reviewing old PRs: Migrated newUserForm.js to MUI #1726 (Unmerged) and update question and date picker #1804 (Merged).
• Created a fresh PR to avoid merge conflicts.

I fixed the heading style on "Welcome!".

Hi @bconti123 - thanks for submitting this PR!

A couple of small bits of feedback:

• Can you please change the welcome text to be larger (should be larger than "Tell us a little bit...."), and reduce the size of "Tell us a little bit about yourself" text. This way text is more easily differentiated.
• Please add more padding between the Text Fields (above & below) so that the helperText still has some space a when text has been input and the helperText moves to the top of the text field.
• Please ensure that the helperText does not overlap the outline of the Text Field when it moves to the top of the text field (currently some of the HelperText text overlaps the outline)

Once this is complete, can you please add some updated screenshots for review?

One final minor fix - can you please change the text on these two lines from grey to black text so that the typographic hierarchy remains consistent for the user?
